FACT is a program of Central New York Health Home Network (CNYHHN, Inc.) that provides fast-track referrals and connections to essential services for individuals and families with low to moderate service needs.
With a supportive team approach, FACT ensures individuals receive quick access to community resources, health services, and social support programs—all without requiring insurance coverage.
FACT is available to adults and children of all ages (birth through adulthood) and serves individuals across Cortland, Herkimer, Madison, Oneida, Onondaga, and Oswego Counties.
