Empowering Health Through Care Coordination
Central New York Health Home Network, Inc. (CNYHHN) is a non-profit organization designated by the New York State Department of Health to provide Health Home Care Management Services. Since 2014, we have served individuals with complex medical, mental health, and substance use conditions, ensuring that they receive coordinated, high-quality care from community-based organizations and specialty medical providers.
Our services ensure that all medical and behavioral health needs are addressed to prevent more significant health conditions over time. Today, CNYHHN employs over 60 Health Home Care Managers, serving both adults and children across Oneida and Cayuga Counties. As one of 32 lead Health Homes in New York State, we also partner with more than 30 organizations specializing in various populations, providing care management services across a 10-county region, from the Mohawk Valley to the North Country and the Capital Region.
