A **Care Management Agency (CMA)** is a **community-based organization** that partners with CNYHHN to provide **Health Home Care Management services**. Care Managers work closely with individuals to: Develop a customized care plan tailored to their medical and social needs Connect them to healthcare providers, behavioral health specialists, and social services Ensure ongoing follow-ups and coordination between different care providers Advocate for housing, employment, education, and financial stability resources --- ## Who Can Benefit from CMA Services?
