What is a Care Management Agency?
A Care Management Agency (CMA) is a community-based organization that partners with CNYHHN to provide Health Home Care Management services. Care Managers work closely with individuals to:
Develop a customized care plan tailored to their medical and social needs
Connect them to healthcare providers, behavioral health specialists, and social services
Ensure ongoing follow-ups and coordination between different care providers
Advocate for housing, employment, education, and financial stability resources

Health Home Care Coordination

1. Find Out If You Qualify
The first step in receiving Care Management services is determining if you meet the eligibility criteria for Health Home enrollment.
2. Submit a Referral Form
If you or a loved one qualifies, the next step is to submit a referral form so we can match you with a dedicated Care Manager.
3. Speak with a Care Manager
After your referral is processed, a Care Manager will be assigned to you. Your Care Manager will:
- Work with you to create a personalized care plan tailored to your health and social needs
- Assist in scheduling doctor’s appointments, mental health support, and community services
- Provide ongoing support and check-ins to ensure you’re receiving the care you need
