How FACT began...... No Wrong Door- An Integrated System of Care
The No Wrong Door initiative was launched in 2019 with funding from the Central New York Care Collaborative- DSRIP. It was developed through a partnership among three community-based organizations including an outpatient public mental health clinic, a lead health home, and a federally qualified health center in Utica, NY. The partners came together with a shared goal: to build a coordinated system that seamlessly connects Medicaid members to the prevention services they need such as behavioral health, care management and primary care services to avoid unnecessary emergency room visits, prolonged hospital stays and improve overall health outcomes.
A Memorandum of Understanding established a shared commitment among the agencies to use a universal screening tool to assess service needs, provide co-located staff for immediate engagement, and ensure closed-loop referrals. This created a streamlined and seamless care process where members were connected regardless of which agency door they first approached.
The initiative’s success led to No Wrong Door receiving the 2021 Collaboration of the Year Award from the Central New York Business Journal and subsequent funding from the Mother Cabrini Health Foundation, allowing the model to expand to serve additional at-risk populations.
Our FACT program design originated with the No Wrong Door initiative.

FACT is a program of Central New York Health Home Network (CNYHHN, Inc.) that provides fast-track referrals and connections to essential services for individuals and families with low to moderate service needs.
With a supportive team approach, FACT ensures individuals receive quick access to community resources, health services, and social support programs—all without requiring insurance coverage.
FACT is available to adults and children of all ages (birth through adulthood) and serves individuals across Cortland, Herkimer, Madison, Oneida, Onondaga, and Oswego Counties.
