Central New York Health Home Network (CNYHHN) provides free, voluntary Health Home Care Management Services for eligible Medicaid recipients with chronic conditions.
Our program connects individuals to medical, behavioral health, and social support services to ensure they receive the care they need.
Eligibility Criteria
For Adults (18 and Older):
To qualify, an individual must be enrolled in Medicaid and meet at least one of the following conditions:

A child must be enrolled in Medicaid and meet at least one of the following conditions:

If you or a loved one is enrolled in Medicaid or Medicaid Managed Care, consider the following:
- Do you or your child have chronic medical or mental health conditions requiring ongoing care?
- Have you experienced multiple emergency room visits or hospitalizations in the past year?
- Do you have difficulty managing medical appointments and treatments?
- Are you struggling with housing, employment, or access to essential services?
